A PEEP
trial shows that a PEEP of 16 cm H2O achieves a balance between oxygen
delivery, lung compliance and hemodynamic performance. (See card: PEEP Trial)
A repeat
CT scan of the abdomen shows that the pancreatic abscess has increased
in size which explains the recent deterioration in gas exchange. Despite
surgical drainage and debridement and broad-spectrum antibiotics, oxygenation
remains a problem. All attempts to reduce either PEEP or FiO2 fail. A
CT scan of the chest demonstrates significant consolidation of both lungs.
