A PEEP trial shows that a PEEP of 16 cm H2O achieves a balance between oxygen delivery, lung compliance and hemodynamic performance. (See card: PEEP Trial)

A repeat CT scan of the abdomen shows that the pancreatic abscess has increased in size which explains the recent deterioration in gas exchange. Despite surgical drainage and debridement and broad-spectrum antibiotics, oxygenation remains a problem. All attempts to reduce either PEEP or FiO2 fail. A CT scan of the chest demonstrates significant consolidation of both lungs.

How would you proceed?

  1. Prone ventilation
  2. Exogenous surfactant therapy
  3. Almitrine
  4. Prostaglandins
  5. Nitric oxide
  6. Continue current treatment
