You have
been impressed with case series using inhaled prostacyclin (PGI2)in patients
with ARDS and call your friendly pharmacist to get some. Unfortunately it
is not in stock and it will take at least 24 hours to get some, however
she does have some PGE1 in stock. You vaguely remember that PGE1 has been
used in ARDS and try a dose intravenously. Unfortunately Mrs. M's blood pressure
drops to 65/40. Intravenous fluid only has a marginal effect on restoring
her pressure and she needs increased support with vasopressors. In addition
with fluid resuscitation oxygenation worsens so that FiO2 has to be increased.
