Although several studies report that the prone position improves oxygenation in 50 - 75 % of patients with ARDS, when you turn your patient from the supine to prone position, her oxygen saturation only marginally improves (88% to 91%). Her oxygen saturation does not improve further despite turning her every 6 hours. (See card: Prone Ventilation)

She is currently on pressure-controlled ventilation with PEEP of 16 cm, distending pressure of 18 cm H2O, FiO2 of 90% and a ventilatory rate of 20 per minute.

What do you do?

  1. Do not change anything at present and wait for a late effect of the prone position
  2. Add nitric oxide inhalation
  3. Give up
  4. Inverse ratio ventilation with I : E ratio of 1.5 : 1
