Aiming
for optimal gas exchange you are pleased to see that pushing PEEP to 20
cm H2O increases oxygen saturation to 96%. However, your patient has become
hypotensive again. Hemodynamic indices show that her cardiac output has
fallen to 4.8 L/min. Realizing that one must balance the positive effects
upon gas exchange with the negative effects upon cardiac index, you decide
