Problems with anti-cytokine therapy

• the two key pro-inflammatory mediators (TNF-a and IL-1) are present for only short periods of time

• most agents are designed to bind a single mediator present in the circulation

• TNF-a and IL-1 may be only indirectly responsible for tissue injury
  • may need to down regulate more proximal cytokines
