Immediately
after insertion of the needle into the 2nd intercostal space on the right,
you hear deflation of air. Her airway pressure and oxygen saturation improve
and you proceed with insertion of a right sided chest tube. (See card: Pulmonary Barotrauma)
In view of some significant pressure ulcers on her forehead and the difficulty turning the patient with a chest tube in situ, you feel it is safer to discontinue prone ventilation for a while. NO-inhalation therapy at 10 ppm is continued. She is still on FiO2 of 55% with a PEEP of 10 cm H2O.
