Unfortunately, experimental animal studies suggest that you might be promoting lung injury by using little or even no PEEP. The concern is that PEEP below the lower inflection point on the pressure-volume curve causes some areas of the lung to open on inhalation and to collapse on exhalation (cyclical atelectasis). It is felt that this directly leads to lung injury.
